Please complete this form if you would like the ACC to establish a Facebook page and user account for the sport program at your school. In order to complete the form you must have:

• Principal approval to establish a Facebook page
• Created a new email address to use with the Facebook account
• A mobile phone number to be used to verify your Facebook page
• A name for your Facebook page e.g. Sport at Trinity College

If you have not already actioned any of these points then you will not be able to complete this form.

Once your Facebook page is established it will become your page and you will be responsible for maintaining the page and checking for inappropriate comments.
