St Sebastian Fellowship nominations can only be submitted by ACC Principals, Sports Coordinators and the Director of Sport. Nominations must also be seconded by another ACC Principal or Sport Director. Selecting YES confirms that you meet the authority criteria and have a seconder to support the nomination.

Nominations should include three digital photos of the nominee. Photo attachments can be any image file or compressed zip file. Photos can also be emailed to the ACC Director of Sport if they are not available at the time of completing this form.
