Soccer Nomination Form

To nominate for the ACC Soccer Championship please complete the following on line nomination form.

When you have completed the form print a copy for your records and then press the submit nomination button to send the nomination to the ACC. Once you press submit a confirmation message should appear on the screen. If you do not see a confirmation screen please contact the ACC via email to check if your nomination was submitted successfully.

* MAXIMUM of 3 teams across indoor and outdoor carnivals. MAXIMUM of 1 team for INDOOR CARNIVAL If you wish to nominate more than the above, please tick the option at the bottom of the nomination form ' . . . if the option were available, would your school like to nominate additional teams'. An ACC staff member will contact you.
TEAM 1 NOMINATION*
Choose the appropriate level of competition for your team: Indoor (entry level) or Outdoor (participation and competitive pools)
Please enter your teams name (be creative !)
NOTE : for a OUTDOOR COMPETITIVE team nomination ONLY, please provide total number of inclusive students PLUS the total number of peer support students
ie 8 inclusive + 2 peer support = 1 0 total
Please ensure the staff name provided will be on site for the duration of the event.

IF THE OPTION WERE AVAILABLE, WOULD YOUR SCHOOL LIKE TO NOMINATE ADDITIONAL TEAMS?*
If you select yes, the ACC will contact you if there is an opportunity to field more teams after the nomination due date.
PRINCIPAL AUTHORITY*
A condition of participation is compliance with details as outlined in the event Charter and that the school Principal is aware of, and endorses this nomination.
IF YOUR SCHOOL DOES NOT HAVE ENOUGH PLAYERS TO FIELD A TEAM(S), DOES YOUR SCHOOL GIVE PERMISSION FOR YOUR STUDENTS TO COMBINE WITH ANOTHER SCHOOL, TO MAKE A TEAM ?*
