Inclusive Sports Program

Nomination form

To nominate for the Inclusive Beach Carnival please :

* complete all fields as required per below, and SUBMIT

* if you do not have all the required information, please enter TBA and email the information to the ACC as soon as you are able.

* once the below information is submitted, you will receive a confirmation email from the ACC. If you do not receive an email from the ACC, your form HAS NOT been submitted correctly. Please contact Cherie Pirnie (ACC)

NOTE

* the event will see students from various schools mixed to form social groups for the activity element of the day

* please ensure you upload the nomination form, as provided by the ACC

** NOTE: if you have mentor / support students also attending, please do not include them in your participant list, but rather provide a total number attending at the base of your participant list ** Please attach the excel nomination spreadsheet provided by the ACC
** NOTE : there is only ONE beach wheelchair available on site, which is currently offered on a first come first served basis. **
1. Please suggest an activity your school will facilitate during the rotations, and please provide a description (if required).
2. Each school attending the day will be responsible for providing at least one activity for the group rotations (eg sand sculpture, beach volleyball, body boarding)
3. PLEASE ADVISE IF YOU REQUIRE ACCESS TO OR TO BE SET UP NEAR THE WATER'S EDGE
4. It is the responsibility of each school to bring all of the appropriate equipment for each activity.
NOTE : Please be mindful when selecting your activity, with group sizes between 20 - 25 students and therefore your staff and equipment need to cater for such.
** PLEASE ENSURE the name provided will be in attendance, and is the point of contact on the event day **
NOTE : Schools are required to supply a minimum of ONE Surf Rescue Supervisor (SRS)- qualified staff member. Their sole purpose is to support with water safety during the iron man events, and also during the activity rotations.
These staff ARE NOT RESPONSIBLE for the supervision of their own school students across the course of the day.
If the staff member is undecided when submitting the nomination form, please enter TBA in both the contact name and the contact number fields, and contact the ACC once the staff member is confirmed.
Principal Authority*
A condition of participation is compliance with details as outlined in the event Charter and that the school Principal is aware of, and endorses this nomination.
